How does the summary that is sent by email at the end of the year calculated?  Does it go from January 1st to December 31st or a different date?  Does it count in manual activities or does it only do recorded ones.

The end of the year summary is based on the calendar year, so it goes from January 1st to December 31st.  They usually provide a link to it before the end of the year, so if you look at it then, it obviously will only go to that date.  If you go back after the end of the year, it will add in the data from those remaining days.  It does include all entries, including manual entries.
