Currently, segment leaderboards on Strava award KOM/QOM only for the fastest overall effort. While this works well for top performers, many athletes—particularly older users—have little realistic chance of competing for the overall title.
A useful improvement would be to introduce age-group KOM/QOM accolades alongside the existing overall KOM/QOM. For example:

etc.
Strava already allows users to filter segment leaderboards by age group, so the necessary data appears to already exist. Adding visible age-group accolades would provide additional goals for athletes and recognise strong performances within comparable groups.
To clearly distinguish these from the overall KOM/QOM, the age-group accolades could use a slightly different visual style. For example, they could use a different coloured crown icon or badge for each age category while keeping the traditional crown for the overall KOM/QOM. This would maintain the prestige of the overall title while still recognising top performances within age groups.
